Strange guy in the hospital basement

Can anyone figure out what the deal is with the guy with long, dark hair in the hospital basement/dungeon? He's always there, never speaks, but seems to listen to and watch whatever is going on there. When Henry dumped his soda on the nurse, the nurse called to him, "You, with the mop, some help here." He must have gotten to Storybrook with the curse, but who can he be?

his name is Chief. he based on a character from One Flew Over the Cuckoo's Nest

I have been wondering about Nurse Ratchet and Chief for awhile. I knew they were from One Flew Over the Cuckoo's Nest. But why and how did they end up in Storybrook? They have been around since the first curse. It has never been explained as far as I know.

Originally, they were a nod to the Baelfire's alias being Neal Cassady. I think they kept them around and included them here and there just as a fun nod type thing.

After a small amount of research...Neal Cassady is the inspiration behind the main character in One Flew Over the Cuckoo's Nest. So a very difficult connection, to me. I had no idea about that.

There are a ton of Neal Cassady connections...including The Grateful Dead and LOST :)
